Its a matter of nuance I think, you have to have quite a bit of media literacy to understand and appreciate an unhealthy pairing. Adding another thought to this because im in the hannibal fandom where inscest and pedophilia pairs are a LOT less common, you have to be a fucking idiot to watch hannibal and think "Oh my god bryan fuller thinks all of this is okay, he wrote a cannibalistic serial killer toxic yaoi and he thinks all of this is okay and people should actively consume human meat and murder people" If you think that leave the fandom and grow up a little before coming back a better person. Writing isn't inherently supporting, its actually good to write about the gross shit, within reason and morality of course. The second it becomes a reality with real world actions is when there's a problem, and I promise its not the A03 writers you need to be worrying about.

Sometimes it can be both, and sometimes it’s not, but you also have the “fans” who harass the actors and their families over a ship, making them severely uncomfortable. Or they bully the hell out of other non problematic fans when they don’t get their way.
Looking at you, ATLA, Teen Wolf, Harry Potter, Supernatural, and so on. Toxic fandoms are toxic fandoms.
